How long are my points valid on the card?
Points are valid for one year from the date of the last service performed.
In which situations does Luxdentica cover the costs of treatment?
If a tooth, implant, or prosthetic restoration was performed at Luxdentica and you were granted a full warranty on it, you are entitled to a reimbursement of dental costs during its validity period (e.g., if you have severe pain in a tooth that is or was undergoing root canal treatment with us). Please seek the emergency assistance you need at that moment! We will refund the money you spend on emergency care up to PLN 300. To receive a refund, obtain your medical documentation including X-rays from the dentist and submit it to our receptionist along with the completed accident claim form (NW).
What if I forget to take my card with me?
To receive points for the service provided, you do not need to have your I LIKE LUXDENTICA card with you. The program will automatically add points to your account. In case of losing the card, the receptionist will issue a new one with the same identification number. The cost of a replacement card is PLN 20.
Where can I collect the rewards?
You can collect your prizes at our clinic. Once you have accumulated the required number of points and chosen your prize, please report to our reception desk. Do not forget your I LIKE LUXDENTICA card when collecting your prize.
Can minors be members of the program?
Minors can participate in the Program if a parent/legal guardian expresses such a wish by completing the registration document.
